COVERAGES AVAILABLE	PLAN BENEFITS
Group Life* & AD&D	$10,000 to $75,000
Short Term Disability	Up to $750/week
Intermediate Disability	Up to $3,000/month
Long Term Disability	Up to $7,500/month

Fully Insured. All plans are provided through fully insured Multiple Employers Trusts arranged by industry classification.

Rates are Sex Distinct and organized in 5 & 10 year age bands. Rates cannot be increased for any single employer participating in the trust without also increasing all other participating employers in the same industry classification.

Guaranteed Issue Benefits. Specified levels of guaranteed issue benefits are available for plans with six or more employees.

Special 10% Discount on Rates now available for Chambers of Commerce and Multiple Employer Trade Associations who have endorsed our plans to their membership.

Proposals are available from user-friendly software we provide, or through our Regional Offices.

* Life insurance is provided by Monitor Life Insurance Company of New York or Fidelity Security Life Insurance Company.

Availability. The plans described are not available in the following states: CA, FL, and HI.
